5 Bridges Wellness Fairs are a celebration of health resources in the community. Attendees get to experience self-care approaches through local wellness practitioners while enjoying food, fun, music & connection. These family-friendly events have something for people of all ages, abilities and backgrounds who want to explore new ways to help support their own wellness journey!

5 Bridges is person-centered integrative wellness planning that is accessible for most adult and youth populations, including those with risk-factors related to trauma, mental illness, substance use, incarceration, and chronic illness. Evidence-based practices are the foundation of our curriculum and coaching process. We follow these approaches: - Trauma and Grief-Informed - Harm-Reduction Model - Motivational Interviewing (MI) - Health Belief Model (HBM) - Theory of Planned Behavior (TPB) Agency staff and wellness professionals have also found success utilizing the 5 Bridges method. Addressing challenges like work-life balance, stress, burnout, and compassion fatigue allow those who serve to also feel supported. Created by Jacob Moore, Founder of NoStigmas mental health nonprofit, this science-backed system is the result of two decades of research, clinical opinion, lived expertise, and survey data. From inpatient and outpatient facilities, private and public schools, for-profit and nonprofit organizations, we empower staff and clients to build resilience and learn to thrive in life.

5 Bridges primary directive is empowered prevention through comprehensive wellness modalities that create a positive paradigm shift for marginalized people. We endevour to accomplish our mission through these core values and strategic approaches: - Meet individuals where they are and support their growth - Show measurable program outcomes through data-driven approaches - Create a culture of wellness within organizations and communities - Strengthen existing networks through program integration - Bring added value and revenue opportunities to our partners Through 5 Bridges curriculum and coaching, we believe that anyone can discover opportunities to improve their wellness and build resilience through strategies and tools that are accessable and simple to implement.

VISION

We envision an expansive culture of wellness where everyone has access to the knowledge and tools necessary to thrive in life. By empowering organizations, wellness practitioners, and clients with the 5 Bridges system, we can create a worldwide community of total wellness. We aim to be the go-to standard for holistic wellness programming. To accomplish this, we’ve created opportunities for engagement on three levels: 1. Organizational leaders 2. Staff and volunteers 3. Clients and community

APPROACH

Our methodology is person-centered and seeks to meet program participants where they are with non-shaming approaches that help to increase capacity and build resilience, supporting treatment objectives and overall wellness. Participants often report experiencing one or more of the following outcomes: - Implementation of a wellness plan - The creation of a crisis protocol - Establishing or changing primary medical care - Better coordination of care and support - Adherence to treatment plans - Consumption of adequate nutrition and hydration - Goal setting for physical activity - Lowering of resting heart rate - Improved sleep and overall rest - Participation in community service - Adoption of a gratitude practice - Sense of empowerment with regard to wellness - Increase in sustainable wellness practices - Introduce modalities that build resilience - Incorporation of peer support - An increased capacity to thrive in life

Integrative wellness planning is tailored to the individual's needs, preferences, and goals. This personalization helps increase motivation and commitment to therapies, treatment plans, reentry strategies and additional health and lifestyle goals.

INTEGRATIVE APPROACH

The holistic nature of integrative wellness planning ensures all aspects of an individual's life are considered in treatment planning. This includes physical health, mental health, social connections, environment, lifestyle habits, and more. By examining these elements in relation to one another, it becomes easier to identify and address potential barriers to treatment adherence.

EDUCATION & SELF-EFFICACY

Our integrative wellness planning includes educating individuals about their wellness challenges and the importance of adhering to their treatment plans. This can enhance self-efficacy and empower individuals to actively engage in their self-improvement process.

PREVENTATIVE & COORDINATED CARE

Integrative wellness also involves preventive measures. It encourages lifestyle changes and healthier habits that can help manage symptoms and reduce risk, while increasing resilience and program adherence. We encourage the involvement of a team of professionals from various disciplines. This level of coordination can help ensure that potential roadblocks are identified and addressed promptly.

Regular check-ins or follow-ups can help keep individuals accountable for their treatment adherence. This often takes the form of coaching or peer support. This shared experience can help individuals feel less isolated and more understood, which can improve their motivation to stick with their plans.

BOND is the act of connecting to anyone or anything that provides you with some form of support. This can include your beliefs, people, pets, places, objects and even knowledge—your sources of strength.

MOVE is the kinesthetic output of your body. Most commonly, we think of physical fitness, but this can also include  the internal movements that happen within your body all the time, like circulation & digestion.

WHEN YOU RECOVER

REST is any time your body, mind or spirit get to recover and reset. Sleep is our primary source of REST, but we go deeper to examine other approaches, such as time in nature and restorative practices.

GIVE explores how we contribute to others and the world around us in positive ways. GIVE can elevate us to another level and feed our soul, but as with every Bridge, it's important to learn balance and boundaries.
